Also, list all authors (with their highest academic degree), author affiliations, financial disclosures, and correspondence address. The corresponding author is responsible for all revisions and final approval of the proofs. The title page should include the name, address, telephone and fax numbers, and e-mail (if available) of the corresponding author. The manuscript (without any author information), cover letter, title page, financial disclosures, permissions, figures, and tables should be submitted as separate files using Editorial Manager. Please allow a minimum of 12 weeks for a decision to be made.
